Eligibility / Target Population

People 18 years and older living in Halton with serious and persistent mental illness (such as psychosis, schizophrenia, mood disorders, anxiety) resulting in functional impairment and significant disability in their daily living

None - Fees covered with a valid Ontario Health Card

Description of Services

Provides long-term case management to enable individuals with severe and persistent mental illnesses to attain self-sufficiency, stability and integration into the community * team of nurses and social workers provides long term slow paced rehabilitation * services are ongoing as long as client needs support * discharge occurs when the client moves outside Halton, refuses or no longer requires service or demonstrates ongoing violent or aggressive behaviour * partner of the Halton Homes Program, working in collaboration with Summit Housing & Outreach, Support & Housing Halton and Joseph Brant Hospital

Services include:
  • psychiatric assessment and treatment
  • medication and symptom management
  • case management, group and individual counselling
  • education about symptoms and treatments
  • relapse prevention planning and strategies
  • collaboration and referral to community partners
  • family/caretaker education and support
