Case Management Services Assist client in achieving and maintaining recovery through the development of comprehensive care plans * linking with appropriate community resources * supporting clients with system navigation, crisis intervention, life skills, service coordination and psychosocial support
Reintegration Program Transitional housing support to older adults with serious mental health and addiction challenges who are presently in hospital (ED/ALC patients) and require transitional support services prior to achieving permanent housing in the community
