Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctor, nurse, and access to allied health professionals through the family health team

chiropody (foot care)
mental health promotion - group support, psychotherapy, resource counselling, advocacy, and system navigation
Stop For FHT (family health teams) - smoking cessation program
Heart Healthy group program
Healthy You program - weight management and nutrition
