Eligibility / Target Population

Must be medically stable and have the cognitive ability to participate in a group exercise program for 6 weeks * can walk 25 meters, with a walking aid if needed * able to get to and from program

Description of Services

Outpatient clinic serving older adults who live in the community * if the patient lives at home, in a retirement home or a long-term care facility and can come to the clinic, the patient will be seen by a geriatrician or a nurse practitioner who will complete the assessment, make recommendations and be in touch with the patient's family physician with the results of the visit
