Description of Services

Provides short term support and case management to children, youth and their families as they navigate through the mental health system and transition to longer term services

Services include:

community case management
collaboration and referral to community partners (connecting children, youth and their families to the most appropriate service provider, liaising with other parts of the health care system, accompanying clients to their appointments with psychiatrists and mental health workers, etc)
crisis intervention and outreach
in-school support to children and their teachers
