A 12-week program that consists of education and exercise components, and brings together a variety of medical resources and specialties, including Registered Dieticians, Pharmacists, Nurses, Physiotherapists and the Ontario Telemedicine Network to help patients achieve heart health and learn about the importance of physical activity
The program consists of three parts
- Assessment: a physician reviews a patient's medical history, conducts relevant testing and prescribes exercises. After 12 weeks, testing will be repeated, the progress assessed, and further recommendations provided
- Education: is available in-person and virtual and focuses on nutrition and medication management
- Exercise: participation in the exercise program should be done in consultation with the health care provider
Patients recovering from an acute cardiac event are referred to the program by a physician
