Description of Services
Self-management, prevention and education rehabilitation program for patients and family members who have been diagnosed with a coronary artery disease * cardiovascular rehabilitation team includes cardiac rehabilitation therapists, clinical pharmacist, medical doctor, psychiatrist and registered dietitian
Services offered include:
medical evaluation
nutritional counselling
cholesterol management
pharmacy counselling
heart health education
stress management
spouse and family support
support to stop smoking
personalized exercise assessment
supervised exercise classes at Recreation, Athletic and Wellness Centre, University of Toronto, 1825 Outer Circle, Mississauga, L5L 1C6
