In-home supports for the activities of daily living to maintain interdependence * services include:

personal care and companionship (assistance with personal hygiene, showering, and grooming)
housekeeping assistance (assistance with light housekeeping, laundry, vacuuming, kitchen, bathroom, and grocery shopping)
respite caregiver relief (in-home relief for informal caregivers, socialization, companionship, medication reminders, personal support, and meal assistance)
transitional care (providing post-hospital care, medication management, support transitioning from hospital to home, and delivery of medical/safety equipment)
home health and mobility aids (wheelchairs, walkers, and scooters)
home accessibility (hospital beds, lift-chairs, porch ceilings and stairs lifts)
bathroom safety (bath chairs, grab bars, and toilet safety frames/risers)
health and wellness (compression garments, bracing and supports, lymphedema management)
