Interdisciplinary team assesses and facilitates support based on health and function required for daily living * referral to appropriate services

Services available:

interdisciplinary team that consists of two social workers, an occupational therapist, a registered nurse, and a physician medical advisor
option to be seen in personal home, retirement residence, or hospital
works collaboratively with other community partners
support discharge for complex alternative level of care patients
