Eligibility / Target Population

Older adults with function or cognitive impairment being discharged from hospital * must be medically stable and able to transfer independently

Description of Services

No charge community support services to promote successful discharges from either in-patient hospital stays or emergency departments * provides individuals to access services for a 6 week period to regain strength and independence * services include:
  • hot or frozen meal delivery
  • transportation for 3 follow up medical appointments
  • homemaking services - 3 visits within 6 weeks
  • home fall safety assessment
  • weekly wellness check ins for isolated referrals
  • service navigation
  • respite (for patients with cognitive decline/diagnosed dementia)
