Description of Services

Free community support services to promote successful discharges from either in-patient hospital stays or emergency departments * Provides individuals to access services for a 4-6 week period to regain strength and independence.
Services may include:

frozen meal delivery
transportation home from hospital
transportation for 2 follow up medical appointments
homemaking services
home safety assessment
wellness check ins
communication with other support services
access to a 24/7 support line
