Specialists offer short-term intensive case management, connection/ reconnection to primary care, and improved coordination of care for patients/clients with complex health needs and experiencing challenges related to the social determinants of health.

A partnership between WoodGreen Community Services, St. Michael's Homes and COTA, Comprehensive Care & Integration (CCI) Specialists support patients/clients with complex health needs to facilitate transitions, fill gaps in care, connect all existing and new providers involved and assist the group to work as an integrated team.
