Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible

National health care organization providing a broad range of more than 50 different programs and services, aimed at improving the health and quality of life of older adults and adults with disabilities

Family Supports Program Support and respite to people with disabilities and their families
Homecare and Professional Services Community health, support and in-home services by nurses and personal support workers
Passport Program Community participation supports and respite supports for adults with intellectual disabilities * plans for support based on each person's needs and goals * help arrange services eligible for the funding including:
