Description of Services
Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
- Home At Last Identifies older adults in local hospitals who have limited supports upon discharge
- Home Help Program Assistance with daily chores, such as laundry, light housekeeping, errands, grocery shopping, and meal preparation * home helpers visit weekly, bi-weekly, monthly (every 4 weeks)
- SMART Exercise and Fall Prevention Program for Seniors Classes are instructed by volunteers and staff trained in the VON SMART® (Seniors Maintaining Active Roles Together) program.
- Volunteer Visiting Support program that matches volunteers with a senior or adult with a disability for one-on-one visits in the client's home or a weekly chat over the phone * visits are regular and may also include enjoyable activities outside of the home
- Wellness Link Programs to support independent living for seniors and adults with disabilities are offered at London Housing Corporation rent-geared-to-income apartments
