People between the ages 14 to 35 years old who are experiencing symptoms of psychosis and who have received either no previous treatment or 6 months or less of treatment for psychosis and who live in the Region of Halton

Description of Services
Outpatient, community-based program for individuals 14-35 years old experiencing early stages of psychosis * clinical partnership between Halton Healthcare, Joseph Brant Hospital, North Halton Mental Health Clinic, Halton ADAPT and the Schizophrenia Society of Ontario * this recovery focused program helps individuals achieve their goals at work, at school and within their social networks, thereby reducing the impact psychosis can have on one’s life * team includes psychiatrists, nursing, social work, occupational therapists, peer support and addiction counsellors

- medication and symptom management
- case management, group and individual counselling
- family education and support
- relapse prevention planning and strategies
- addictions counselling
- peer support
- community program referral and illness management instruction after discharge
- assistance with education, work, and housing
