Through home and office visits, and phone support, the program nurses assist patients in understanding:
The CHF program also has a direct affiliation with GHC’s cardiac rehab program * CHF patients can be referred to the exercise component (VIPEX) as well as the services of the registered dietitian and social worker
- signs and symptoms of CHF
- importance of medication compliance
- diet and fluid restrictions
- importance of exercise
