With any of: cognitive impairment or difficulty walking/transferring/recent falls ; Emergency department use in previous 30 days or in hospital previous 90 days ; 5 or more medications ; Emergency Department identify risk(s) - suspect abuse, neglect, pain management, failure to cope, caregiver strain, difficulty with activity of daily living, medication issues (non compliance) ; lives alone, no caregiver, lives in long term care or retirement home
