Description of Services
Medical care team focused on family and community health
- Offers timely access to high quality primary health care
- Coordinates documentation if patient receives care from a member of the team
- Provides access to other health care professionals within and outside of the Family Health Team
- Administers various health programs
- Chronic disease management
- Diabetes program
- Health promotion
- Disease prevention
- Nutritional counselling
- Mental health clinic
- Provides access to a Registered Nurse who provides advice about urgent health care concerns
- Offers the opportunity to have family doctor receive a written summary of the call
