- Adults 18 years of age and older with physical and developmental handicaps
- Adults with mental illnesses or acquired brain injury
- Seniors in aged 55 and over in a low income category

Description of Services
Provides the following support services for individuals living with a serious mental illness or mental health concern
Intensive Case Management Program
Intensive Case Management Program
- Provides flexible, individualized and ongoing support with case management
- Develops individualized support plans to assist with personal recovery goals
- Provides specialized services to adults living with a developmental disability and a serious mental illness
- Support services include but are not limited to access to psychiatric, occupational therapy, behaviour therapy, and cultural support services
- Provides programs and motivational support for increased active leisure among vulnerable adults
- Activities may include but are not limited to walking groups, fitness, bowling and cooking
- Provides a monthly food bag consisting of mostly vegetables to program participants
