- have a right to request a review of the decision

Application
- Individuals who require financial assistance right away should apply for assistance through Ontario Works as applications can take anywhere from a week to a few months to be processed
- Individuals can apply online, by phone or in person
Eligibility / Target Population
Individuals must be between 18-65 years of age and:
Various financial criteria must be met; individuals will have to provide information on the following:
Disability criteria is based on the Ontario Disability Support Program Act and must meet the definition of a "person with a disability" as defined under the Act which states this individual is someone who has a substantial mental or physical disability that:
- be a resident of Ontario
- meet both the financial and disability criteria
Various financial criteria must be met; individuals will have to provide information on the following:
- household composition (who is part of the family, spouse or adult dependents)
- how many children, how old they are and whether they are in school
- place of residence and cost to live there (rent)
- household income (for all members)
- assets, such as any savings in the bank and any property owned
Disability criteria is based on the Ontario Disability Support Program Act and must meet the definition of a "person with a disability" as defined under the Act which states this individual is someone who has a substantial mental or physical disability that:
- is continuous or recurrent
- expected to last for a year or more
- significantly limits their ability to work, look after themselves, or get out in the community, and
- has been verified by an approved health professional (Medical Review)
