Provides services and supports to assist individuals to achieve and maintain a quality life
- Assists individual in developing a realistic plan to acquire the skills and supports to enable them to live independently
- Determines areas of need and develops the support plan with a focus on each individual
- Provides referrals to community resources and supports
- Provides the supports in a community setting or within individual's home environment
- Focuses on wellness and recovery
- Daily living skills, basic needs and life and social skills
- Employment
- Navigating the mental health system
- Accessing medical and psychiatric services
- Medication and symptom management
- Crisis prevention, intervention and management
