Supports for individuals living with pre-diabetes or diabetes
- Provides Certified Diabetes Educators to help individuals manage their diabetes
- Assesses, monitors, and assists individuals with diabetes management and to attend to any diabetes complications they may have
- Offers education and self-management support to fit an individual's lifestyle
- Complication/Co-morbid condition management
- Insulin starts and adjustments
