Description of Services

Supports for people living with prediabetes or diabetes
  • Offers education for prevention and self-management support to fit an individual's lifestyle
  • Provides access to a registered nurse and registered dietitian
  • Provides referrals to other health care professionals
  • Provides supplies to manage diabetes
Programs and services include:
  • Blood sugar, HgA1c, blood pressure and oxygen monitoring
  • Client support groups
  • Community screening
  • Foot care workshops/clinics
  • Health fairs
  • Presentations to the public and to health care providers
  • School programs and visits
Ontario Telemedicine Network (OTN)
  • Provides access to specialized medical care using video conferencing
  • Allows specialists to remotely examine and prescribe treatments to patients in their home communities
  • Reduces the need to travel to receive medical care
