Description of Services
Integrated stroke care * information and resources for stroke survivors * promotes healthy living and stroke prevention with community partners
Care provided from the emergency room and inpatient setting, to rehabilitation and return to the community * works with partners in long-term care
Care provided from the emergency room and inpatient setting, to rehabilitation and return to the community * works with partners in long-term care
- Integrated Stroke Unit 13-bed acute and rehabilitative care program for people who have had a stroke * 5 acute care beds and 8 rehab care beds * services provided by doctors, nurses, personal support workers, physiotherapists, occupational therapists, speech language therapists, dietitians, pharmacists, and social workers
- Stroke Prevention Clinic Stroke prevention for patients within their community, who are at high risk for stroke, have had a Transient Ischemic Attack (TIA) and/or who have had a non-disabling stroke
