Description of Services
One-stop point of access of Ontario Health atHome in Mississauga Halton for:
With one phone call, a team member will help determine eligibility for these programs and connect with the care needed or refer to alternative resources that may better meet the needs
Serving the communities of Oakville, Milton, Halton Hills, Mississauga and South Etobicoke to:
- Caregiver ReCharge (for informal caregivers of loved ones 18+ years of age)
- Supports for Daily Living (for high risk seniors, 65+ years of age)
- Oaville Senior Citizens Residence (OSCR) Short Stay Respite (out-of-home caregiver relief for high-risk adults, from 2 nights to 2 weeks, and up to 3 times per year)
- Food Support Programs (Meals on Wheels and Food Advisor Supports)
- Adult Day Programs (part-day recreational and social activities for 18+ years of age with physical disabilities, frail elderly or people with dementias)

- assess client eligibility for these programs based on standardized criteria
- prioritize incoming referrals
- assign clients the most appropriate service provider to meet their needs
- track and trend the demand for services
- maintain a centralized waitlist for services
