The Transitional Care program provides individual and group programming to individuals who require support and education regarding their mental illness and to provide seamless and timely transition from inpatient to community-based care. Services provided include assessment, psychoeducation, supportive counselling, life skill development as well as healthy lifestyle programming and a sheltered transitional employment opportunity.
