Eligibility / Target Population
- Individuals requiring physiotherapy services
- OHIP coverage for those 19 years of age and younger, 65 years of age and older, ODSP or Ontario Works recipients, and individuals hospitalized for at least one night due to an injury

Description of Services
Hospital based community physiotherapy and occupational therapy clinic
- Health professionals assess and treat conditions that affect the muscles, nerves and bones that may be caused by work or sports or motor vehicle collision injuries, chronic conditions, or to recover after an operation (hip and knee surgery)
- Home visits are available as needed
