Multi-disciplinary team includes physicians, nurses, and social workers *
team works with patient's primary health care team to provide
consultation services for inpatients and their families * team helps
with:
- education to patients and their families
- end-of-life management
- relieve suffering
- assistance with future care planning
- discharge planning
