Application
Informed consent must be given (includes knowledge of and understanding of medical diagnosis, available forms of treatment, available options, including palliative care), a written request must be submitted, medical assessments are required and a ten-day waiting period is required * additional provincial requirements may be necessary
Eligibility / Target Population
Must be:
- eligible for publicly-funded health services in Canada
- 18 years of age or older
- mentally competent, including in health care decisions
- diagnosed with a grievous and irremediable medical condition
- voluntary requesting medical assistance in dying without outside pressure or influence
- able to give informed consent

Medical assistance in dying is available for people residing in Canada who have a grievous and irremediable medical condition * drugs that end life are either directly administered by the physician or nurse practitioner or prescribed and then taken by the patient themselves
