Referral required from member of patient's health care team (social workers, case managers, charge nurses, therapists, physicians or discharge planners) at Northumberland Hills Hospital or Campbellford Memorial Hospital * fax referral form
Eligibility / Target Population
Residents of Northumberland County who are older adults or adults with disabilities who have little to no supports in place on day of discharge from the hospital * a patient being discharged from a Northumberland County hospital

Program to help make the transition from
hospital to home on the day of their discharge
- driving or accompanying a client home
- picking up medications or groceries
- meal preparation
- personal care or homemaking services
- follow-up phone call to check on their well-being
- referrals to other community support services if necessary
