Internal - Submit referral form (Acute to Rehab and Complex Care) to Intake Office through Meditech

External - Submit referral form by fax

People 16 years and older with an acquired brain injury who display challenging behavioural and/or mental health issues * must have physical and cognitive abilities that will allow for active participation in a rehabilitation program * must be reasonably likely to achieve stated functional goals within estimated hospital length of stay * must be medically stable with no significant fluctuation in medical status within 72 hours before admission * must have a viable community discharge plan or repatriation agreement
