For Health Service Providers - Centralized intake process through Hamilton Health Sciences Intake Office

Internal - Submit referral form (Acute to Rehab and Complex Care) to Intake Office through Meditech

External - Submit referral form by fax

Refer to "Regional Rehabilitation Referral Process and Admission Criteria"

Description of Services

Hospital inpatient acquired brain injury rehabilitation program

Community Re-integration Unit - Rehab care for adults with moderate acquired brain injuries to receive functional life skills training for community re-integration

Slow to Recover Program - Rehab care for people who have had a severe acquired brain injury * cares for patients who are not able to access services in their home communities

Average length of stay is 62 days
