Hospital-based outpatient rehabilitation program for people who have had a stroke, brain injury, spinal cord injury, or other new neurological event

• helps patients re-learn activities of daily living
• helps with swallowing difficulties
• help with memory and thinking skills
• educates them about their health
• connects them with other community services

Treatment lasts up to 12 weeks maximum * Progression level of rehabilitative care for patients working to restore or maximize functionality
