Transitional clinic for people living with atrial fibrillation
Services include:
Services include:
- timely access from ED or Primary Care Provider to the first clinic visit
- education about AFIB and its management
- access to necessary cardiac investigations
- comprehensive management of AFIB and other comorbidities
- timely access to cardiology, electorphsiology, or specialty care if needed
- seamless transition to community care providers with 24-48 hours of visit
