Services offered:

Family practice medical services - involve a multidisciplinary healthcare team that includes family physicians, nurse practitioners, and registered nurses. Services include:

• one-on-one medical appointment for individual clients
• well baby visit
• immunization for infants and young children
• adult health maintenance and management of chronic diseases
• pregnancy care for women who are up to 27 weeks pregnant
• counselling for mental health and substance use disorders
• chronic disease management- diabetes education, physiotherapy, chiropody, registered dietitian services
• special health education activities and programs for clients and community members (pre-registration may be required)
• case management and system navigation
• low vision clinic (In collaboration with Vision Loss Rehab Canada)
