In-home 24-hour personal care program * helps older adults who are hospitalized, or at high risk of hospitalization, successfully return, or safely remain, in their own home
- medication monitoring
- assistance with the activities of daily living
- bathing, dressing, grooming
- basic homemaking
- laundry
- shopping
- meal preparation
- light housekeeping
- safety checks
- daily 24 hour urgent response
