Eligibility / Target Population
Case management services are available to people 16 years and over who have been diagnosed with a mental illness including
* schizophrenia
* manic depression
* chronic depression
* severe anxiety disorder
* OCD
Client must also have difficulty managing daily life and have experienced long term difficulties with the illness
