Application

Must call the care coordination service to provide necessary information * a doctor or nurse practitioner will determine if you meet the eligibility requirements

Eligibility / Target Population

  • Must be 18 years of age or older
  • eligible for publicly funded health care services in Canada
  • be able to provide informed consent
  • have a grievous and irremediable medical condition
  • capable of making health care decisions
  • voluntarily request MAID
