Any person who has a life-threatening or life-limiting illness and requires end of life support (no age restrictions) * any person, family or friend who is experiencing or has experienced loss and is in need of support

- end-of-life resident care
- individual palliative support
- in-home volunteer visiting
- caregiver support
- information, referral and health system navigation
- Advance Care Planning
- Grief and bereavement counselling and support groups

A multi-service, volunteer-based community support organization that operates programs within Northumberland County which act to aid both clients and their caregivers. Programs operate from six local offices within the County. Call the local Northumberland Community Care office to inquire about program criteria and to pre-register for services.
