Description of Services
Offer a wide range of outpatient services including clinics that offer assessment, diagnosis/treatment and therapy for patients and members of the community
- Adult Asthma Clinic Multi-disciplinary health professionals providing patients with education and health care to better manage their condition of asthma
- Anticoagulation and Thrombosis Clinic Specialized outpatient clinic that provides assessment, support and education to patients with confirmed conditions requiring anticoagulation, including Venous Thromboembolism and Atrial Fibrillation
- Cardio Respiatory Offer respiratory diagnostic procedures and therapeutic interventions * specialize in the diagnosis of lung diseases such as asthma, chronic obstructive pulmonary disease (emphysema) or pneumonia * breathing test and pulmonary function test * assessment of sleep apnea
- Cardiology Follow-up Clinic Asses, diagnose and treat cardiac-related issues including atrial fibrillation, congestive failure and hypertension
- Chiropody Clinic Services focus on the diagnosis, treatment and prevention of acute disorders of the foot to patients at a high-risk, for ulcerations, infections and amputations
- Clinic for Healthy Aging Initial and follow-up assessments, health education, consultation, diagnosis of dementia including Alzheimer’s Disease, treatment and assistance in planning for the future
- COPE(Osteoporosis) Clinic Assess and test patients who have been diagnosed with osteoporosis to optimize treatment and self-management to maintain bone health and preventing fractures
- Dental Clinic Dental surgeon provides on-site dental treatment to individuals who may have underlying medical condition
- Heart Failure Clinic Interprofessional team assesses and recommends treatement for patients diagnosed with heart failure * team consists of a Cardiologist, Registered Nurse, Registered Dietician, Registered Pharmacist, Physiotherapy and Social Worker
- Outpatient Rehabilitation (Progression) Hospital-based outpatient general and orthopedic rehabilitation program * services provided by physiotherapists and occupational therapists
- Pacemaker Clinic Outpatient clinic that provides education and follow-up for patients with implanted pacemakers * pacemakers are checked by a Device Certified Technologist and reviewed by a Cardiologist
- Unity Health Toronto - St Joseph's Health Centre - Ambulatory Care Centre - Sleep Lab Sleep technologists provide sleep study testing to monitor and assess sleep disorder symptoms
