Description of Services

Diabetes education for adults, families, and caregivers aimed at effective self-management * culturally sensitive diabetes services and education programming * people living with type 1 and type 2 diabetes, prediabetes, and gestational diabetes are welcome

Services include:
  • individual assessment to identify diabetes self-management and learning needs
  • individual appointments with diabetes nurse educators and registered dietitians
  • group sessions
  • self glucose monitoring
  • insulin initiation education
  • insulin pump program
  • interpreter services
