Apartments with 24-hour personal care
- light housekeeping
- laundry support
- meal preparation and support
- reassurance and security checks
- medication reminders
- access to transportation and support for weekly grocery shopping trip
- emergency response button
- social and recreational programs
- must live on location – TCH Building – 7/11 Arleta Ave
- Case Management Services Assist client in achieving and maintaining recovery through the development of comprehensive care plans * linking with appropriate community resources * supporting clients with system navigation, crisis intervention, life skills, service coordination and psychosocial support
- Reintegration Program Transitional housing support to older adults with serious mental health and addiction challenges who are presently in hospital (ED/ALC patients) and require transitional support services prior to achieving permanent housing in the community
