Visit website * contact local Home and Community Care Support Services or call 310-2222 for all questions, including eligibility and long-term care homes information

Eligibility / Target Population

All applicants must be 18 years of age or older * have a valid OHIP card * have specific care needs * have care needs which cannot be safely met in the community through publicly-funded community-based services and other care-giving support * have care needs which can be met in a long-term care home

Description of Services

Works to reduce waitlists and to ensure that the long-term care sector has a path forward to handle the growing elderly population * visit website for any information in regards to long-term care homes

To ensure this, the ministry works to :
  • deliver better coordinated care in the community and closer to home by improving quality of life in long-term care homes
  • ensure Ontario’s healthcare system is able to seamlessly transition older adults through various levels of care
  • work with the Ministry of Health and partners to reduce long-term care waitlists across the province
