Description of Services

Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals provided to other local health services and personal development groups

Family practice medical services involve a multidisciplinary healthcare team that includes family physicians, nurse practitioners, and registered nurses 

Services include:
  • one-on-one medical appointment for individual clients, in-person and virtual visits
  • interprofessional primary care for the African, Caribbean Black population
  • well baby visit
  • immunization for infants and young children
  • adult health maintenance and management of chronic diseases
  • pregnancy care for women who are up to 27 weeks pregnant
  • mental health and substance use disorder services
  • chronic disease management - diabetes education, physiotherapy, teleophthalmology-diabetes eye screening, foot care
  • sickle cell disease
  • sexual health clinic
  • mental health counselling
  • registered dietitian services
  • case management and system navigation
  • mobile health care (Wheels to Wellness) at specific locations in the community
  • special health education activities and programs for clients and community members (pre-registration may be required)
