Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes Nurse Practitioners, Nurse (RPNs & RNs), Registered Dietitians, Pharmacist, Social Workers, Psychologist, Occupational Therapist, Health Promoter, Case Manager, Receptionists
Group and 1:1 services available for chronic disease prevention, chronic disease self-management, and mental health supports.
Rapids Family Health Team's Access to Care Centre offers same-day/next-day appointments with Nurse Practitioners for acute & episodic health concerns (fever, infection, rash, sore throat, minor injury, etc)
